PLEASE NOTE: If your Adobe software package is “Adobe Acrobat Pro,” and if it is older than version 9.0, you may experience difficulty in saving and editing portions of these forms.

MACINTOSH USERS: Most Macs will default to a program called Preview when opening PDFs. PTI’s forms will malfunction in Preview. To prevent such malfunction, download Adobe Reader to your computer using the free link below. Then download PTI’s forms to your desktop, and, when you wish to open them, use the “open with” command to select Adobe Reader.
