Grantee Reporting Forms
The reporting forms below are for use by current grantees. The Interim and Final report forms will expand to accommodate text. The Financial Report PDF is fillable and will add figures within columns automatically.
MACINTOSH USERS: Most Macs will default to a program called Preview when opening PDFs. PTI’s forms will malfunction in Preview. To prevent such malfunction, download Adobe Reader to your computer using the free link below. Then download PTI’s forms to your desktop, and, when you wish to open them, use the “open with” command to select Adobe Reader.
